Defective DMD chip, send me return form please

Dear Picopix Max Support
Now have a growing white dot on screen, projector shows no dust nor any apparent damage and is in normal use, so seems a defective DMD chip issue. Have received it less than 2 years ago so is still under guarantee. Please send a return form for replacement ? Have also emailed screeneo support but no answer yet. Not sure how to proceed, please help
